FUNCTIONAL DUTIES:
- Primary Care Services:

- Identify risk factors, health needs, and develop individualized care plans.

REQUIREMENTS:

- Current and unrestricted Nurse Practitioner (NP) or Registered Nurse (RN) license.
- Certification as a Family Nurse Practitioner (FNP) or relevant specialty.
- Prescriptive authority and DEA registration (for FNPs).
- Strong clinical assessment and diagnostic skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a team-based primary care setting.
- Commitment to continuous learning and professional development.
- Knowledge of electronic health record (EHR) systems.
